To some degree it reminds me of the one-page report we would get from our kids when they were in daycare (like ages 2-4) where it would show their mood:

- happy

- grumpy

- bitey

And so on...

Also, I have a neighbor down the street who supports a certain "American" football team. When they are having a bad day I always joke to my wife...

"Honey, should I walk down the street and do a Wellness Check?".

I'm planning to add this for this season. You can choose your "mood" and add a comment after a session is over.

This is not completed or formatted how I want yet (probably 9-12 options) but here's the general idea:
